    Plenty of times.
    Once when I was really little, my cousin and I were having a race across a parking lot and I just pedalled as fast as I could and just ran into the curb and flipped over.

    Technically no. Though, when I was little, I was playing in the pool with my friend and we wanted to see if she could stand on my shoulders in the deep end (we weren't that bright). So, she went on my shoulders, but eh water was shallow enough that she was pushing me down. I started feeling suffocated and tried to push her off, but she thought I was just playing. So, I ended up clawing at her, her finally getting off my shoulders, and me freaking out when I came to the surface. That's about as close as I've gotten to death.

    Yep, my used to only have a cassette deck, so I made mix tapes. Also, my dad used to be a DJ, so I grew up on them.

    Ran through a glass door? No, that sounds painful. I watched a friend of mine walk into a pane of glass that she thought was an open door, once.
